Cross-Border Collaboration: Strengthening Outbreak Preparedness (2026)

A critical cross-border initiative took center stage in November 2025, aiming to fortify our defenses against vaccine-preventable disease (VPD) outbreaks. The event, a collaboration between WHO India and WHO Nepal, brought together Surveillance Medical Officers (SMOs) in Gorakhpur, India, and Biratnagar, Nepal, to enhance preparedness and cross-border cooperation.

But here's where it gets controversial: with a shared border spanning approximately 1800 kilometers, the region faces unique challenges in outbreak response. And this is the part most people miss - the key to success lies in building a robust network of SMOs who can provide technical expertise and ensure high-quality VPD surveillance at the subnational level.

The meetings focused on equipping SMOs with the necessary tools and strategies to tackle priority vaccine-preventable diseases along the border. Participants delved into the International Health Regulations (IHR) and emergency response frameworks, gaining insights into global standards for public health emergencies. They also received updates on standard operating procedures for responding to poliovirus events, measles, and rubella outbreaks, ensuring a unified and swift response to cross-border incidents.

A highlight of the event was a tabletop simulation exercise (TT SimEx) on polio and measles outbreaks crossing international borders. This exercise provided a platform for participants to share management strategies and strengthen cross-border collaboration, a crucial aspect of addressing health threats in the region.

The overarching goal of these meetings was to foster better cooperation between the WHO country teams, ultimately enhancing the region's ability to prevent and control VPD outbreaks. By strengthening partnerships and aligning strategies, the region can better safeguard public health along the border and beyond.
